Trainee's Information
Please make sure that all information is correct because we will be using these for your certificates and IDs, Onergy records, and reports to be submitted to DOLE-OSHC. Double check the spelling before proceeding to the next part.
Training Package Shipping Information
Please indicate the following details for the delivery of your training package (certificate, ID, and freebies):
- Complete address (street address, barangay, city/municipality, province, other landmarks)
- Recipient name
- Recipient contact number
Payment Instructions
- You may opt to pay in full (Php 4,000), or pay a reservation fee of Php 500 (non-refundable).
- Payment (full or partial) shall be made after filling out this registration form.
- After sending your payment, please email us the proof of payment to payments@onergy.ph.
- Any remaining balance shall be paid on or before the first day of training.
NOTE: Your slot will only be secured after you have made the payment (full or partial) and sent the proof of payment to the email indicated above.
